Submitting Claims and General Information

Required Documents for Claims Payments: 
  • Completed and Signed Claim Form (To print a claim form, select the link below. You will be redirected to the Renaissance Agency website where you will be asked to select your school/organization. Claim forms are available on the school/organization information page Claim Form )
  • Itemized Bills (receipts of payments alone cannot be accepted)
  • Referral from Student (Campus) Health Center - if required (see plan brochure)
  • Certificate of Prior Creditable Insurance Coverage (should be submitted with or prior to first claim submission). See plan brochure if required.

For faster processing, providers can submit claims electronically using Payer ID 95397.
