Required Documents for Claims Payments:

Completed and Signed Claim Form (To print a claim form, select the link below. You will be redirected to the Renaissance Agency website where you will be asked to select your school/organization. Claim forms are available on the school/organization information page Claim Form ) Itemized Bills (receipts of payments alone cannot be accepted) Referral from Student (Campus) Health Center - if required (see plan brochure) Certificate of Prior Creditable Insurance Coverage (should be submitted with or prior to first claim submission).
