| Many student health insurance
plans incorporate cost containment features designed to provide the best care
available by providing access to a panel of medical professionals, typically
known as a Preferred Provider Organization (PPO). This network of physicians,
clinics, medical practices and hospitals, have agreed to discount their
charges. |

| To find out if your
plan uses one of the PPO networks, please refer to your student health
insurance brochure or your ID card. Once you know which network you belong to,
select the link below to access the PPO website to view important information
about the network and to search for a listing of contracted providers. |

treatment from a network provider, be sure to inform them that you are a
subscriber of the PPO network identified in your brochure or on your ID card. |

be aware that if an Insured is treated at a PPO hospital, it does not mean that
all providers at the hospital are PPO providers. In addition, if an Insured is
referred by a PPO provider to another facility, it does not mean that the
provider or facility to which the Insured is referred to is also a PPO provider |
