If you need to submit a claim for an intercollegiate sports-related accident or
injury you will not be able to use this online service. To submit a claim for
an intercollegiate sports-related accident or injury, please contact your
athletic department.

claim personal information that we maintain. Theses pages are secured
with 128-bit SSL encryption. The identification information required for access
to these pages is internally validated to ensure that this information cannot
be viewed without proper authority.
Any attempt to circumvent security measures would be in violation of state
and/or federal law. Those found in violation will be prosecuted to the fullest
extent of the law.
